• Systolic HF, HFrEF – EF = 40% or less
• Diastolic HF, HFpEF – EF = 50% or more
• EF = 41-49% – HFmrEF (heart failure with mid range EF) – There is no code for this. The providers can decide if systolic or diastolic.
° ACUTE CHF ( decompensated or exacerbation=acute)
– Exacerbation of symptoms- rales, edema, dyspnea
– BNP=100 or Pro-BNP>300
– IV Lasix
– Supplemental O2
° Cardiogenic shock – SBP < 90 for 30 minutes or more with signs/symptoms of shock
° Chronic
– CHF > 3 months and on meds for heart failure
